Eleven million Canadians are living with diabetes or prediabetes.
High blood sugar levels in your body, if not well-managed, can damage organs, blood vessels, and nerves, leading to other complications, such as heart disease.
It’s important to get tested if you have risk factors, and to know if you have prediabetes, because there is an opportunity to manage your condition before it progresses into Type 2 diabetes.
Black Creek CHC offers education for adult clients with pre-diabetes and Type 2 diabetes. A nutritionist teaches you to eat well, overcome stress, and stay on track toward health goals to prevent the development and advancement of the disease.
If necessary, a Chiropodist (Foot doctor) will assess your foot health for diabetes-related foot issues, as well as other foot conditions and injuries.
Eye Screening Program
Although nearly two million Canadians living with diabetes can experience vision loss, early detection and treatment can reduce the risk of blindness by 95 per cent.
Through a collaboration with the Ontario Telemedicine Network (OTN) Black Creek CHC’s team offers eye screening for patients with diabetes at various locations throughout the Greater Toronto Area.
Please note that your eyes may be sensitive after the assessment. Please make arrangements for someone to drive you home after your appointment.
No referral required.
