The Diabetes Prevention Challenge

Connect with a Community Health Coach!

If you are at risk for Diabetes, connect with a health coach to get started on your wellness journey.

We are accepting referrals for clients and community members:

  • At risk of developing Type 2 Diabetes
  • Live/Work in Northwest Toronto / Black Creek – Humber River
  • Ready to participate in health coaching to improve health
  • Participants will receive coaching for approximately 4 months on a weekly basis by a community health coach
  • Ability to use a digital device such as a cellphone
 
Digital Equity: We recognize that some people experience barriers to accessing digital devices or cellphones. If you have concerns about your ability to access a device in order to participate, please let us know. 

Community Health Coaches

Community Health Coaches use their skills and experience as trust-builders to help connect people with the services and support they need to reduce their risk of developing diabetes.

Health Coaches help community members live healthier lives by:

 

  • Uncovering motivations
  • Setting SMART goals
  • Supporting through challenges
  • Celebrating progress

 

Health Coaches will use the NexJ Connected Wellness platform for health coaching

 

  • Addresses whole person health (physical, mental, emotional, social, financial, and community wellbeing)
  • Personalizes health coaching (Individualized content, and a dedicated health coach)
  • Supports behaviour change (supportive tools & resources to successfully develop healthy habits)
  • Engages clients in their health (To achieve realistic and sustainable lifestyle changes)
  • Is backed by clinical evidence (Over 25 clinical trials and 30 peer-reviewed publications)

IMPACT Canada Type 2 Diabetes Prevention Challenge

  • Over 3 million Canadians, or 8.9% of the population have been diagnosed with Diabetes.
  • Type 2 Diabetes accounts for 90% of Diabetes cases in Canadian adults.
  • Some risk factors are attributed to the conditions where people grow, work, live, and age, and are dependent on social, economic, and environmental determinants of health. 

The Public Health Agency of Canada (PHAC) is calling upon community innovators to co-design care approaches between now and March, 2025 to address the community’s risk of developing Type 2 Diabetes.
