Black Creek Community Health Centre is committed to the protection and responsible collection, use and disclosure of personal information, in compliance with privacy legislation.
Black Creek Community Health Centre does not collect personal information from visitors who access and browse through our website. The only personal information captured through our website is the information that you share with us such as when you register for a group program, or when you share your email address with us to receive information. We will not disclose or share this information for any other purposes. We do not install software programs on your computer or device.
The handling, retention, and destruction of users’ information is in compliance with data protection laws. Black Creek CHC retains user information for as long as it is necessary to provide services but will destroy the information when it is no longer needed or when requested by the user.
Use of website cookies
Our website uses Cookies, which are used to help improve the content within our site by helping us understand how visitors use our site. The data that is collected is anonymous and includes statistics about how our website is being accessed, what pages are being viewed, and time spent on the website. Cookies do not collect any identifiable information about you.
Information Sharing
Black Creek Community Health Centre does not sell any information that is collected via its website or social media accounts. We do not disclose any identifiable information about a client unless we have their express consent, or when required by law.
Email Subscriptions
As per Canada’s Anti-Spam Legislation (CASL), individuals must provide their express consent to be added to Black Creek CHC’s email distribution lists. To be removed from an email list, individuals may unsubscribe, or contact Black Creek Community Health Centre and request to have their name removed from any distribution lists.
Third Party Links
Black Creek CHC’s website includes links to other third-party websites. The organization will ensure that posted links are up to date and functioning, but does not assume any responsibility for the information practices of websites accessed through our site.
Your Personal Health Information
Your name, address and date of birth are your personal information. So is all information regarding your physical and mental health, your health history, your family’s health history and any care or services you access.
Language preference
For Francophones, your language preference is also considered to be personal health information.
Accessing your personal health information
Your Care Team members, who are collaborating to provide you with care, are allowed to access your personal health information.
If you are being referred to a specialist, pharmacist, or another care provider, outside of Black Creek Community Health Centre, it may be necessary to share your personal health information as part of this process.
How do we use and share all or parts of your personal health information?
We may use and disclose all or part of your personal health information to:
- Care for and treat you
- Get payment for your treatment and care (from the Ministry of Health and Long Term Care, OHIP, IFH, or others)
- Plan, administer and manage our internal operations
- Conduct quality improvement, professional development activities and accreditation review process
- Teach students within the Centre
- Conduct research
- Compile statistics
- Comply with legal and regulatory requirements, and
- Fulfill other purposes permitted or required by law.
How can you access your personal health information?
Each client has the right to see and correct their personal health information. You can do so through a written request. Information will be provided within 30 days of your request.
What rights do you have with regard to your personal health information?
When you are asked to provide personal health information, you can either do so or refuse to provide the information. You can also control some of the uses and sharing of your information.

The Ontario Personal Health Information Protection Act (PHIPA) requires us to have clear policies and practices on personal health information and to post this statement.
