What is PACT?
People Accessing Care Teams (PACT), provides access to a team of health professionals either at the health centre or elsewhere in the community.
In 2018, Black Creek CHC expanded the Inter-Professional Care Team to include health services most needed by residents living in Toronto’s Northwest neighbourhoods.
Services offered through PACT include:
- Chiropody Assessment and Screening – Foot Care
- Chronic Obstructive Pulmonary Disease (COPD) – Lung Health Program (Spirometry, Smoking cessation, exercise)
- Community Registered Dietitian
- Community Supports and Referrals
- Diabetes Education and Self-Management
- Diabetic Retinopathy Screening – Teleopthalmology Program (TOP)
- Harm Reduction – Supports and Referrals
- International Board Certified Lactation Consultant – Breastfeeding Support
- Physiotherapy (Non-MVA)
- Registered Kinesiologist
- Social Work/Therapist
- Sexual Health (Walk-in)

Who can access PACT?
Anyone with a referral from a physician or nurse practitioner can receive care through PACT. Care from the Inter-Professional Care Team is offered at no cost to the client, regardless of their health insurance status.
