International Youth Day Family Field Day Event

Saturday Drop-in Forms Clinic
Do you need more information about childhood immunizations?
Is your child facing school suspension?
Visit our Drop-in Forms Clinic and we can help you learn more about immunizations and steps for updating your child’s immunization records.
We can also help out with other forms, including: Ontario Disability Support Program (ODSP), Ontario Works (OW), and Social Housing Applications.
